Could you please verify that you've only receive on yearly payment? My phone provider is saying I paid 3 times.

Hey thanks for posting your question. It looks like you subscribed via the Google Play Store, so they actually handle the payment, however, it should only allow you to sign up for one Premium account per Google Play account. Please do follow up with us though if you see more than one on your Google Play account or ccard statement.
